Living Well at Home

Living Well at Home is a 3-week programme designed specifically for carers of patients who are known to us.

Over the 3 week programme we will cover a range of topics to provide guidance, education and support for you to cope with any worries or pressures you may be facing. The programme aims to provides a safe an environment where you can meet other carers and share experiences with each other.

The sessions will be held at the Turner Centre, at St Michael’s Hospice. These will be formatted in a group setting.
– Monday afternoons – 2pm – 3:30pm

It is encouraged that you attend each session, to enable you to have the best understanding and support, however it is not essential.

You will be contacted by the Living Well Service nurse prior to attending the programme, this is to answer any questions or concerns you may have.

The programme is detailed below:

Week 1: ‘Resilience and Managing Stress’
We will look at what coping strategies you may already have in place, how you deal with situations as they occur, recognising carer fatigue and provide advice for managing stress.

Week 2: ‘Resources and Practical Support’
A chance to cover some of the practical things that may arise when caring for someone. We will provide information and education on equipment, falls prevention, benefits and support available.

Week 3: ‘What to Expect When Someone is Nearing the End of their Life’
This week is an opportunity to talk about what may happen as your loved one becomes more unwell, how you will know when that is happening and what to do when it happens. We will include information around the dying process.
